The Dementia Directory supports people living with dementia to better access care the care they need to live well in the community for as long as possible.
Primary health networks were funded by the Australian Government in 2022 to develop dementia resources as a response to the Royal Commission into Aged Care Quality and Safety. This directory of local services and support is for people living with dementia, their carers and families. It was developed to complement existing directories for My Aged Care and NDIS, and is for people who live in the northern, western, eastern and central suburbs of Melbourne. Other local dementia directories are also available for Victoria and Tasmania.
